Usually the simplest answer is the correct one - There is a good chance you picked up some nasty intestinal parasites on your travels - I'm sure you only drank bottled water - but often the culprit can be ice cubes or fresh fruit washed in unsafe water - you also bought from a lot of street vendors with unknown sanitation habits. Parasites would explain the continued bouts of diarrhea, weight loss, tiredness, sleepiness, and lack of appetite along with several other symptoms - Hopefully, your doctor will run more tests and find a cure soon - then you will be your happy healthy self once again.
